BRENTWOOD PEDIATRICS, PLLC: USING OUR PATIENT PORTAL
Each child will have their own Patient Portal. The user names are the child’s first name and first 4 digits of the date of birth. For example, if the child’s name is Samuel born 01/05/2001 the user name would be samuel0105. The password is password. You will be prompted to change the password once you are in the Patient Portal.
To complete the questionnaires for annual well visits: Once you are in the patient portal, look to the left side under “Questionnaires” and click on “Medical History.” Click onto your child’s age of exam, answer the questions, and submit. This feeds into your child’s chart.
If you are unable to get the questionnaires completed online, you will need to arrive 10 minutes early for your appointment. However, we cannot stress enough that your pediatrician prefers that these questionnaires be completed online.
To request a refill/referral: Once you are in the patient portal, look to the left side under “Messages” and click either “Refill Request” or “Referral Request.” Please follow the instructions and submit. This comes into your child’s record and a clinical employee will communicate with you via either the portal or by phone. If requesting a refill, be sure that your pharmacy is listed. If requesting a referral, be sure to include what type of referral you are requesting.
For non-urgent physician communications: Once in the patient portal, look to the left side under “Messages” and click “Ask a Doctor,” follow the instructions, and submit. This comes into your child’s record and a clinical employee will communicate with you either through the portal or by a phone call. Please be sure to include a phone number that you can be reached at.
To view lab results or other information from an office visit: Once you are in the patient portal, look to the middle right of the screen under “Medical Records” and click on “View Medical Records.” This takes you to the Personal Health Record. If you scroll down you can view your child’s problem list, most current vital signs, a medication list for your child, lab results, and immunizations.
To view previous or in-process referrals: Once in the patient portal, look to the left side under “Medical Records” and click “Referrals.” You will be able to click on a referral entry and view who your child will be seeing, when the child’s appointment is, and why your child is being referred.
In the future, we hope to be able to publish school notes/forms, immunization records, patient education, and payment processing.
